Before Your Appointment

First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by completing or providing the following at the time of your consultation:

• Your surgical referral slip and any X-rays if applicable
• A list of medications you are presently taking
• If you have insurance, bring the necessary information. This will save time and allow us to help you process any claims.
• Pre-registration and view patient information video

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.

Before your initial appointment please assist us by following these steps.

• Step 1) Pre-registering with our office:
  
  Please complete the forms available through our secure online Patient Registration Form. After you have completed the forms, please make sure to press the submit button at the bottom of the page to automatically send us your information. On your first visit to our office, we will have your completed forms available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

IMPORTANT: All patients under the age of 18 or age 18 and still in high school must be accompanied by a parent or legal guardian at the consultation visit.

• Step 2) View patient information video:
  
  Our online videos are presented in order to help you understand more about the oral surgery procedures we perform. Please watch the video specific to your appointment.

• Step 3) X-rays & referral slip:
  
  If your dentist or physician has taken x-rays, you may request that they forward them to our office. If additional films are necessary, they can be taken at our facility. Please bring your referral slip to our office at the time of your appointment.
  
• Step 4) Appointment confirmation
  
  Expect an appointment reminder from our office via automated House Calls System approximately 2 days prior to your scheduled appointment. Please contact our office if you will need to reschedule or cancel your appointment 24 hrs before your scheduled appointment.
